Sigma Physiotherapy wants you to be confident that the personal data we hold for you is safe and secure. We collect your data in a number of ways and it is kept confidential at all times.​ This document outlines what information we hold and where and what your rights are.

“Us”: Sigma Physiotherapy Ltd, a private Limited Company by guarantee without share capital use of 'Limited' exemption registered in England and Wales (Company number:  12774194) having its registered address as 48 Wayland Road, Sheffield, S11 8YE (the ‘Discloser’)

 

“You”: Patients who are currently or have previously received services provided by Sigma Physiotherapy etc  or those enquiring regarding about receiving services

The type of personal information we collect  

We currently collect and process the following information about people who engage with us: 

  • Name 

  • Address 

  • Email address 

  • Telephone number 

  • Date of Birth 

  • Medical history

  • Treatment notes

  • Number of sessions provided

  • Communications such as emails or SMS

How we get the personal information and why we have it 

Most of the personal information we process is provided to us directly by you for one of the following reasons: 

  • To help us in providing a service to you. 

  • To form a contract for the service you are purchasing. 

  • To get in touch with you to provide information about our services or to form part of the service that you have bought. 

  • To understand your medical history so we are better able to help you. 

We also receive personal information indirectly, from the following sources in the following scenarios: 

  • Your GP: We may need to contact your GP for certain information about any conditions you present to us. 

  • If you have a Carer, it is helpful to receive information from them about the problems you encounter. 

  • If you have a Power of Attorney that helps you make decisions or makes decisions on your behalf, we will consult with them to enable us to give you the best care possible. 

 

We use the information that you have given us in order to understand your medical conditions and history, so we are better able to help you. We also use the information you have given us to stay in contact with you.  

We may share this information with your GP, Carer or Power of Attorney if we feel it will help you to get better. We will not share this information with anyone else unless a child is being abused or a life at risk. 

 

Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:  

(a) Your consent. You can remove your consent at any time. You can do this by emailing us at hello@sigmaphysiotherapy.co.uk

(b) We have a contractual obligation. 

(c) We have a legitimate interest.  

 

How we store your personal information  

Your information may be securely stored on:  

  • Acuity Scheduling 

  • Xero 

  • iCloud.com 

  • One drive and Microsoft products

  • Google suite and google products  

  • Meta (Whatsapp, Facebook, Instagram)

  • Mailchimp

  • Square 

  • Wix

We suggest that you make sure you are happy with the Privacy Policies of the above companies. We do not assume responsibility for the manner of use or misuse of information made available by you on these other websites. We advise you do not provide personal information without assuring yourself of their Privacy Policy Statements. 

Payments 

Payments for our on Premises services are taken through a card machine either when you are in clinic or prior to your first appointment on the phone. When we take your payment details over the phone, we do not keep or store them. When payments are taken in clinic, we are not exposed to, nor do we store any of your financial information.  

​Who has access to your data 

Your data is secured securely but in the nature of their work our team and/or contractors require access to your data. 

Prior to working with us all team members sign a non-disclosure agreement and they work inline with our confidentiality policy. 

Retention 

We keep all information that we collect about a patient for: 

  • Eight years from the date of last treatment for adult records.

  • Eight years after their 18 birthday or until 25 years of age for children.

​We will then dispose your information by deleting the digital files we have stored.

Cookies 

We put small files (known as cookies) onto your computer to collect information about how you browse the site. You can opt out of these.

 

Your data protection rights 

Under data protection law, you have rights including: 

Your right of access - You have the right to ask us for copies of your personal information.  

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.  

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.  

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.  

Your right to object to processing - You have the right to object to the processing of your personal information in certain circumstances. 

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances. 

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
